Abstract     Despite signs and symptoms of peripheral autonomic imbalance attributed to parasympathetic overactivity and ocular sympathetic paralysis, several observations locate the primary pathogenetic mechanisms of cluster headache centrally to the hypothalamus and central pain control regions. Cluster headache is associated with dilatation of proximal branches of the internal carotid artery without consistent changes in cerebral blood flow. No important changes have been reported in endocrine function or circulatory humoral agents.
