Abstract     Background. In the introduction a short historical review of hormonal replacement therapy (HRT) is presented. Follow the definitions of menopause, peri- and postmenopausal periods and of the climacteric period. According to literature the average age at menopause is about 50 years in developed countries while in Slovenia it is 47.7 years which corresponds to the age at menopause in developing and underdeveloped countries. Clinical manifestations of pre- and postmenopausal periods are considered as emphasizing the interplay between estrogen deficiency and the occurence of cardiovascular diseases (CVD) as well as osteoporosis. Methods. Preventive and therapeutic effects of HRT on vasomotor symptoms, CVD and osteoporosis are discussed, taking relative risks for endometrial and breast cancer into consideration. Conclusions. All types of available HRT are presented, including estrogens, gestagens, estrogen-gestagen combined preparations and those for local application. Some alternatives to estrogens for menopausal symptoms are included assuming that perimenopausal woman should be managed by a critical and experienced physician.
