Abstract     Background. Of all benefits of hormone replacement therapy (HRT) a reduction in the incidence of cardiovascular diseases has the greatest potential to influence life expectancy of women. Morbidity and mortality from coronary heart disease rise sharply after menopause. Methods and results. Meta-analysis of epidemiological studies have shown an average decrease by 40 to 50 per cent in coronary risk among estrogen users compared to non-users, while there is not enough data about cardiovascular protection with either nonoral route of estrogen replacement or combined estrogen-progestogen replacement. The mechanisms mediating the cardioprotective effect of oestrogens are favourable changes induced in the plasma lipoprotein profile and in arterial wall itself. Conclusions. The decision about HRT usage for prevention of coronary heart disease should be based on global coronary risk of individual woman; it is indicated in women with premature menopause, in women with symptomatic coronary heart disease and in women with high coronary risk. Oral route of estrogen replacement has advantage to alternative forms of estrogen delivery since cardioprotective effect of this form of application has already been shown in epidemiological studies.
