Abstract     Background. Laparoscopy has only recently been introduced in the treatment of borderline ovarian cancer (BOC) as a modality equivalent to open surgery. This retrospective study compares the results of laparoscopic and laparotomic treatment for BOC. Patients and methods. We analysed retrospectively 111 patients with BOC, operated on at the Department of Gynaecology, University Medical Centre Ljubljana, between 2000 and 2009. Preoperative clinical data (gynaecological ultrasound and serum CA-125 determination), and data on intraoperative procedures and postoperative follow up were retrieved from medical records and analysed. The patients were divided into two subgroups based upon whether they were primarely treated by laparoscopy or by laparotomy. Results. The laparoscopic group comprised 53 (47.7%) patients with BOC; primary laparoscopy was definitive treatment in 20 (37.7%) patients; ten (18.9%) patients had conversion to laparotomy for final surgical steps. In 24 (45.3%) patients the decision to postpone surgery was made. Nine patients underwent laparotomy, five had laparoscopically assisted vaginal hysterectomy and ten had laparoscopy. In the laparoscopic subgroup, surgical treatment was completed laparoscopically in a total of 35 (66.0%) patients. The laparotomic subgroup included 58 (52.3%) patients. Postponed laparotomy was performed in four (6.9%) cases. Postoperatively, none of the patients treated received any adjuvant treatment. During a three-year follow-up period following primary surgical treatment, 103 (92.8%) patients attended for ultrasound and serum CA-125 determination twice a year. During the follow-up period, six (5.4%) patients- five (9.4%) from the laparoscopic group and one (1.7%) from the laparotomic subgroup - underwent a second-look operation. Recurrence was confirmed in three cases, i.e. in 2.7% of all patients treated for BOC. (Abstract truncated at 2000 characters)
