Abstract     In the following paper we present a female patient with rapidly progressive glomerulonephritis due to connective tissue disease, most likely systemic lupus erythematosus. The course of illness is described from the first visit to nephrologyst, through all the diagnostic procedures, treatment and progression to the end-stage renal failure. We want to emphasize the importance of recognition of early abnormalities in urine analysis in connection with clinical presentation. In acute nephrytic syndrome with progression of renal failure early refferal to nephrologyst is mandatory. In this group of patients rapid diagnostic procedures (foremost kidney biopsy) need to be followed by appropriate treatment. Rapid institution of therapy is key to successfull treatment and long-term prognosis.
Summary     V prispevku je prikazana bolnica s hitro napredujočim glomerulonefritisom v sklopu sistemske bolezni veziva, najverjetneje sistemskega lupus eritematozusa. Opisana je celotna obravnava bolnice od prvega prihoda v nefrološko ambulanto do postavitve diagnoze, zdravljenja ter spremljanja napredovanja bolezni do končne ledvične odpovedi. Povdarek je na zgodnjem prepoznavanju urinskih sprememb v kombinaciji s klinično sliko, saj je v primeru nefritičnega sindroma z napredujočim slabšanjem ledvične funkcije nujna čimprejšnja napotitev k nefrologu za ustrezno nadaljnjo diagnostiko in zdravljenje.
