Abstract     Background. Cardiogenic embolism accounts for one in six ischemic strokes. Atrial fibrillation in the absence of rheumatic valvular heart disease, which affects 5 to 12 per cent of the population over the age of 60, is the single most important characteristic associated with cerebral embolism. The risk of stroke in this population is 5 times that of comparable population without atrial fibrillation. Other underlying conditions connected with cardioembolic events are: mitral and rarely aortic valvular heart disease, prosthetic heart valves, acute myocardial infarction, left ventricular disfunction, infectious endocarditis and congenital heart disease. Conclusions. Long term anticoagulant therapy with warfarin is highly effective in preventing stroke in cardiac patients. Low intensity regiment with prothrombin time international normalised ratio (INR) from 2.0 to 3.0 may be sufficient prophylaxis for most embolism-prone cardiac disorders. Only in patients with prosthetic heart valves INR should be 2.5 to 3.5.
