Abstract     Atherosclerosis is a change in arterial wall. Imbalance between input and output of cholesterol-rich lipoproteins and lack of antioxidants produce localinflammation, formation of atheroma, and thrombosis. Features of the pathobiology of the atherosclerotic plaque, in addition to its degree of luminal encroachment and localization, influence the clinical manifestations of atherosclerosis. An enhanced understanding of plaque biology provides insight into the diverse ways in which atherosclerosis can present itself clinically and why the disease may remain silent or stable for prolonged periods, punctuated by acute complications at certain times. This article describes the pathophysiological mechanisms that lead to atherosclerosis development and progression and present the targets for future clinical intervention, and finally, emphasizes the role of prevention.
Summary     Ateroskleroza je sprememba v steni arterije. Neravnovesje med vnosom in iznosom lipoproteinov, ki po telesu prenašajo holesterol, v to steno in odsotnost antioksidantov v njej povzročita lokalno vnetje, tvorbo ateroma in trombozo. Biološke značilnosti in lokacija aterosklerotičnega plaka ter stopnja zožitve svetline arterije vplivajo na klinično pojavljanje ateroskleroze. Razumevanje biologije plakov daje boljši vpogled v različne oblike ateroskleroze in zakaj ta bolezen ostane tiha ali stabilna v daljših časovnih obdobjih z vmesnimi akutnimi zapleti. Pričujoči prispevek opisuje patofiziološke mehanize, ki vodijo v nastanek in razvoj ateroskleroze in ki predstavljajo tarče za klinične posege prihodnosti, ter izpostavlja pomen preventive.
