Abstract     The 5alpha-reductase (5AR) type 1 and 2 enzymes catalyze the reduction of different 4-ene-3-oxosteroids (including testosterone) into the most potent androgen, 5á-dihydrotestosterone. Increased activities of the 5ARs are related to different diseases, such as benign prostatic hyperplasia, androgen alopecia, hirsutism and acne. The 5ARs are thus emerging drug targets for treatment of 5á-dihydrotestosterone-dependent diseases. Here, we review the major classes of 5AR inhibitors. Of many 5AR inhibitors reported over the last four decades, the only finasteride and dutasteride are used in the therapy of benign prostatic hyperplasia, and finasteride also in the therapy of androgen alopecia. Due to their serious side effects and the need for more effective and selective inhibitors, the search for inhibitors continues. Over the last few years, the discovery of new non-steroidal inhibitors has gained great importance, as these compounds have fewer interactions with receptors or with other enzymes of the steroid endocrine system.
