Abstract     Background: The presence of sensory, cognitive and perceptual deficits often increases the severity of functional disabilities, causes emotional distress, interferes with the patient's ability to participate in and learn during rehabilitation, and increases the risk of injury. The aim of our study was to assess the prevalence of sensory, perceptual and cognitive deficits of stroke patients admitted to comprehensive rehabilitation programs and their impact on rehabilitation outcomes. Methods: Retrospective study of frequency of sensory, perceptual and cognitive deficits registered in medical documentationand of rehabilitation outcomes of stroke patients admitted to the stroke rehabilitation unit of the University Rehabilitation Institute in Ljubljana from January 1st to June 30th, 2011. Results: Eighty per cent of stroke patients had sensory deficits, and 65 percent had cognitive and perceptual deficits; 20 percent of the latter had severe communication problems. The rehabilitation outcomes of patients with cognitive and perceptual deficits were worse than those of patients without deficits, though they still showed statistically significant improvement of functioning. Conclusion: The assessment and documentation of sensory, cognitive and perceptual functions is not sufficiently systematic. We need to supplement screening measures to evaluate cognitive and perceptual functions. To improve our therapeutic approaches of cognitive and perceptual deficits we need to reform our stroke rehabilitation programs emphasizing occupational, speech and language and psychological therapies.
Summary     Izhodišča: Motnje zaznavnih in spoznavnih sposobnosti zvečajo stopnjo funkcionalnih omejitev, povzročijo čustvene stiske, težave pri sodelovanju in učenju v času rehabilitacije ter povečajo nevarnost za poškodbe. Iz medicinske dokumentacije bolnikov, sprejetih na rehabilitacijsko obravnavo po možganski kapi, smo želeli ugotoviti pogostnost zaznavnih in prepoznavnih motenj in njihov vpliv na izid rehabilitacije. Metode: Retrospektivna analiza podatkov o motnjah občutljivosti, zaznavnih in prepoznavnih sposobnostih in izidu rehabilitacije v medicinski dokumentaciji bolnikov, sprejetih na oddelek za rehabilitacijo bolnikov po možganski kapi na Univerzitetni rehabilitacijski inštitut Republike Slovenije - Soča od 1. 1. do 30. 6. 2011. Rezultati: Osemdeset odstotkov bolnikov je imelo motnje občutljivosti, 65 odstotkov pa spoznavne motnje, od tega 20 odstotkov hujše težave pri razumevanju govora in govornem izražanju. Izid rehabilitacije bolnikov s hujšimi zaznavnimi in spoznavnimi motnjami je bil slabši kot izid pri tistih brez težav ali z blagimi težavami, vendar je bil napredek pri obojih v času rehabilitacije statistično značilen. Zaključek: Ocenjevanje občutljivosti, zaznavnih in spoznavnih sposobnosti ter zapisovanje ugotovitev v medicinsko dokumentacijo bolnikov še vedno ni dovolj sistematično. Dopolniti bi morali presejalna orodja za ocenjevanje zaznavnih in spoznavnih motenj. Postopkom za obvladovanje zaznavnih in prepoznavnih motenj bi morali posvetiti več časa, za kar pa bi bilo potrebno rehabilitacijske programe preoblikovati, okrepiti delovno terapijo in logopedsko ter psihološko obravnavo bolnikov.
