Abstract     Izhodišča: Z meritvami smo želeli ugotoviti, ali je za osebe po nadkolenski amputaciji hoja s pripomočkom za hojo Femuret bolj naporna kot hoja z nadkolensko protezo s kolenom na zaklep. Metode: V študijo smo vključili deset bolnikov po transfemoralni amputaciji zaradi bolezni žilja s sočasnimi boleznimi drugih organov. Pri šestminutnem testu hoje smo pri bolnikih merili porabo kisika (VO2 ) z direktno metodo merjenja in z aparatom za elektrokardiografijo registrirali srčno frekvenco in morebitne spremembe pri elektrokardiogramu. Izmerili smo povišanje krvnega tlaka med hojo, prehojeno razdaljo v šestih minutah in izračunali hitrost hoje. Bolniki so subjektivno ocenjevali napor pri hoji z Borgovo lestvico zaznanega napora. Učinkovitost hoje smo ocenjevali z metaboličnim in fiziološkim indeksom porabe kisika. Rezultati: Razlike v porabi kisika, maksimalnem povečanju srčnega utripa in zvišanju krvnega tlaka med hojo s pripomočkom in hojo s protezo niso bile statistično značilne. Ugotovili pa smo statistično značilne razlike pri prehojeni razdalji in zato tudi pri hitrosti hoje (p < 0,001). Poraba kisika je bila pri obeh načinih hoje v povprečju 50% predvidene maksimalne glede na starost, maksimalno doseženi srčni utrip pa okoli 70 % maksimalno predvidenega. Hoja s protezo je bila glede na metabolični in fiziološki indeks porabe učinkovitejša kot hoja s pripomočkom Femuret (p < 0,001). Zaključki: Hoja s pripomočkom Femuret in hoja z nadkolensko protezo sta ob individualno izbrani hitrosti hoje energetsko enako zahtevni. Glede na intenzivnost napora predstavljata zmerno telesno aktivnost. Hoja s protezo je zaradi večje hitrosti hoje bolj učinkovita kot hoja s pripomočkom Femuret.Background: The aim of the study was to establish if walking with the early walking device Femuret is more straining than walking with a transfemoral prosthesis with locked knee for patients with transfemoral amputation. Methods: Ten patients with transfemoral amputation because of vascular disease and comorbidity were included in the study. The oxygen consumption (VO2 ) with the direct method, heart frequency and ECG changes were measured during the 6-minute walking test. We also measured blood pressure before and after the test and the walked distance. The velocity of walking was calculated. The patients estimated their effort with Borgs scale. Walking efficiency with metabolic and physiological consumption index were estimated. Results: There were no statically significant differences in oxygen consumption, maximal heart frequency and blood pressure between walking with the Femuret and with a transfemoral prosthesis. Statistically significant differences were observed in walking distance and walking velocity (p<0,001). Oxygen consumption was on average 50% of the maximal predicted with respect to age, and maximal heart frequency about 70 % of the maximal predicted. Walking with a transfemoral prosthesis was more effective in terms of the metabolic and physiological consumption index than walking with the Femuret (p < 0.001). Conclusion: At an individually chosen speed, energy consumption is equal when walking with a transfemoral prosthesis or with the Femuret. Both constitute moderate physical acitivity. Because of higher speed, walking with a transfemoral prosthesis is more effective then walking with the Femuret.
