Abstract     Myocardial Infarction (MI) is a complex multifactorial disorder caused by the interaction of environmental and genetic factors. Several types of genetic studies and approaches, such as family studies, linkage analysis, candidate gene approach, and genome wide association studies, have tried to unravel the genetic background of MI. Although it is clear that the genetic component has an important role in the development of MI, in the view of many all these approaches have failed to deliver as expected. It might be due to the fact that beside genetic factors, gene-gene interactions, and epigenetic mechanisms are most probably very much involved in the setting of MI. For clinical use, a clear cut and reliable information should be available from genetic and epigenetic studies to be of help in the decision making in patients with coronary artery disease.
