Abstract     V članku je predstavljen vpliv kakovostne predaja na varno obravnavo pacienta v Splošni bolnišnici Jesenice. Timska predaja pacienta je ključnega pomena za kontinuiteto obravnave. Pri predaji pacienta pride med strokovnjaki do prenosa ključnih informacij o stanju in oskrbi pacienta. V primeru, da ne pride do prenosa informacij je lahko oskrba pacienta nepopolna, lahko je celo ogrožena varnost pacienta. Predaja pacienta vpliva tudi na kontinuiteto, ki predstavlja stopnjo združevanja različnih nepovezanih dogodkov v zdravstveni obravnavi, ki se kažejo kot skladen in povezan pristop zdravstvenega osebja do pacienta in njegovih potreb in kot dobro medsebojno sodelovanje osebja, ki izvaja zdravstveno obravnavo. Prav to nas je vodilo pri standardizaciji timske predaje pacienta. Namen članka je prikazati standardizirane postopke timske predaje in posveta po viziti. V nadaljevanju so prikazani cilji, katerim zdravstveni strokovnjaki sledijo, predvsem zaradi izboljšanja prenosa ključnih informacij in kontinuitete pri obravnavi pacienta. V naši bolnišnici se zavedamo, da je predaja pacienta pomemben del oskrbe pacienta in predstavlja z vidika kakovosti in varnosti enega izmed ključnih elementov. V nadaljevanju članka je predstavljena analiza anketiranja pacientov o pomenu ob posteljne predaje in njihovega vključevanja v predajo pacientov. Predstavljen je način timske predaje pacienta v bolnišnici, seznanitev vseh članov negovalnega tima o vsebinskem in časovnem izvajanju timske predaje pacienta, o konzultacijah in posvetu o stanju pacientov in pridobivanju drugih informacij, ki so potrebne za neposredno izvajanje zdravstvene nege (ZN) in obravnave pri pacientu na vseh nivojih in prenos odgovornosti za pacienta iz enega zdravstvenega strokovnjaka na drugega. Timska predaja vodi do boljšega skupinskega dela, izboljšuje kontinuiteto zdravstvene nege in izboljšuje rezultate pri pacientih.The impact of quality patient care delivery on the safe treatment of patients in General hospital Jesenice is presented. Continuous health care is of paramount importance for the quality treatment of the patients. Continuity is introducing a certain degree of agglomeration of different unconnected events in the health treatment, that present as harmonious and joined approach of health personnel to the patient and his needs. It also shows a good mutual cooperation of personnel involved in the health treatment. These facts led us to standardize the patient care delivery model. Standard procedures of team delivery and consultation round visit are described. Goals which are followed by health specialist to improve the transfer of crucial information and to ensure the continuity health treatment of the patient. It is kept in mind in our hospital, that delivery of the patient is an important part of patient health care and from the quality and safety point of view, also one of the key elements. Analisys of patients' questionnairs about the meaning of bedside delivery and its inclusion in the patients delivery is presented. The patient care delivery model is presented. All members of the health care team are informed about the content and timing of the team delivery of the patient, about consultations and other information needed for the health care treatment and treatment of the patient on all levels. The transfer of responsibility for the patient from one health specialist to the other is described. The method of introducing the patient care delivery model is presented.
