Abstract     The aim of this study was to determine whether the less aggressive approach of continuous mixed venous oxygen saturation (SvO2) monitoring and increase of oxygen delivery (DO2) with fluids and dobutamine in those injured patients who had decreased Sv02 or positive dobutamine test are more relevant for the survival of multiple injured patients than the automatic maintenance of supranormal values of oxygen transport variables with aggressive fluid and dobutamine therapy. We were also interested to know whether any of the oxygen transport parameters has a prognostic significance. The study comprised 40 adult patients with multiple trauma who were admitted to the Central Intensive Care Unit of the University Medical Centre Ljubljana between September 1989 and May 1993. The group included 33 men and 7 women between 17 and 70 years of age (mean 37.37 years). Seven patients died. All the patients were observed for 5 days after injury. The patients were divided in two groups (A and B). Group A consisted of 23 patients, in whom Sv02 was continuosly measured and D02 increased only if required (study group). The patients had an Oximetrix-Opticath thermodilution catheter inserted in the pulmonary artery for continuous Sv02 monitoring. The dobutamine test was used to detect a possible pathological dependence of oxygen consumption (V02) on oxygen delivery (D02). Group B comprised 17 patients in whom oxygen transport variables were consistently maintained above the normal levels (control group). The patients had a standard Swan-Ganz thermodilution catheter inserted in their pulmonary arte ry; oxygen transport related variables and SvO2 were measured periodically. Groups A and B were compared with respect to survival and frequency of occurrence of multiple organ failure (MOF). We also compared the group of survivors with that of the patients who died in order to determine if any of the oxygen transport variables was of prognostic significance.(trunc.)
