Abstract     Background: The treatment of patients with complete bilateral cleft lip and palate (BCLP) is complex, because normal function must be established and aesthetic goals must also be achieved. Surgical procedures performed in early childhood influence midfacial growth, especially the development of the maxiIla and to a lesser degree of the mandible. By analysing lateral cephalograms, we wanted to determine to what degree the facial skeleton in patients with BCLP treated in Ljubljana with Čelešnik's method differs from that in healthy individuals of the same age and how many treated individuals need orthognathic correction in adulthood. Methods: This retrospective study is based on an analysis of lateral cephalograms from 26 patients with complete BCLP, born between 1978 and 1994. The control group consisted of 26 non-cleft individuals with Class I occlusion. Statistical analysis was performed with Student's t-test. p-values below 0.05 were considered statistically significant. Results: The subjects who had been treated in child hood for BCLP had maxillary and mandibular retrognathism and avertical growth pattern of the facial skeleton with increased inclination angles of the maxiIla and mandible to the skull base. The mean values of the SNA and SNB angles were significantly smaller, while the inclination angles of the maxiIla and mandible were larger than in the control group. Conclusions: Primary surgical treatment of BCLP affects the growth of the maxiIla, and the mandible adapts to the changes. More than 80% of patients treated with Čelešnik's method do not need orthognathic surgery in adulthood.
