Abstract     A comparative study on two different operative techniques for the treatment of the chronic pilonidal disease is reported. From the 102 patients treated by radical excision and primary closure by the Z plastic in the period of five years (from 1985-89), the group of 34 patients with chronic pilonidal sinus was randomly chosen and was compared to randomly chosen group of 34 patients treated by excision and healing by open granulation. The results show a significant difference beetwen the two groups in the lenght of hospital stay, in the number of privious incisions of acute abscess in the time required for the complete recovery and capability to work, in frequency of the complications (secretion, inflamation, seroma) and in recurrence rate followed for up to six years. No recurrence has yet been encountered in the group treated by the Z plastic and no complications with the good function and aesthetic effect occured in all cases. In the group of patients treated by the secundary granulation after excision of pilonidal sinus the recurrence rate was 23,53 percent, the function and aesthetic apperance was satisfactionary only in 50 percent and bad in 5,88 percent of cases. The nest results were obtained in the group treated by the Z plastic. That leads us to the conclusion that this technique is superior method in the surgical treatment of the chronic pilonidal disease.
