Abstract     A 47-year-old patient sought medical assistance because of pain in the lower part of the abdomen. Clinical investigation revealed a 10 cm tumor of the right ovary which was also confirmed by ultrasound. Laboratory investigations showed decreased serum gonadotropin and increased serum estradiol and progesteron levels, while CA 125, AFP, CEA, CA 19-9 and B2M tumor markers were normal, except for SCC which was increased. Color Doppler ultrasound revealed pathologic tumor vascularization with increased diastolic flow and lowest measured RI 0.17. Lower median laparotomy, as well as dextral adnexectomy were performed. Tumor was invading the broad uterine ligament with proliferations on the surface. Frozen section performed intraoperatively revaled a case of granulosa cell tumor of the ovary. The operation proceeded with total hysterectomy, sinistral adnexectomy, omentectomy, as well as pelvic and paraaortal lymphadenectomy. All of the 61 removed lymph nodes proved negative. Postoperative course was normal.
