Abstract     In our retro and prospective study of 60 patients with epithelial ovarian carcinoma the clinical singnificance of preoperative serum CA 125 level was assesed. We obtained sensitivity and specificity for predicting ovarian cancer that of 79 percent and 66,6 percent, respectively. In results, tumor grade had a strong influence on the preoperative CA 125 level, the correlation was high and significant (r = 0,74, p = 2,03E-11). The preoperative CA 125 level had no predictive value for survival in patients with ovarian carcinoma. There was a significant, good correlation between the preoperative CA 125 level and stage of disease (r = 0,57 p = 1,5E-06), but this is, we think, the indirect reflection of tumor grade. We obtained the highest CA 125 values in patients with serous type ovarian carcinomas. The size of primary tumor had no influence on the preoperative antigen level. There were only two parameters with significant influence on survival: stage of disease (r = 0,497 p = 0,018) and residual tumor mass (r = -0,456; p = 0,049). In the same stage of disease there was significant difference in preoperative CA 125 level among different grade of tumor differentiation (p less th. 0,05). There was no significant difference in preoperative CA 125 level when we compared the same tumor grade in different stage of disease and additionally, there was also no difference in CA 125 levels in histologically different, but the same grade ovarian carcinoma. Our opinion is, preoparative serum CA 125 determination is an important diagnostic tool, which preoperatively enables tumor grade estima tion and indirectly also stage of disease estimation.
