Abstract     The choice of primary treatment of malignant tumors of thehead and neck, surgical or non-surgical, depends on factorssuch as tumor type, site and locoregional extension; thepresence of distant metastases; available facilities, personneland experience of the treating center; and patient factorslike comorbidity. Moreover, the choice of treatmentmodalities is based on the expected optimal oncologic outcome weighed against the best quality of life and functionaloutcomes after treatment. In the interest of optimal patient care, we examine thereasons for recommending non-surgical therapies for certainepithelial cancers of the head and neck. Generally, theusual indications for choice of non-surgical rather than surgicaltreatment are (1) for certain histological tumor typesthat respond well to non-surgical treatments, (2) advancedlocoregional disease impossible to extirpate completely orwithout producing an unacceptable functional outcome,
