Abstract     Introduction: To ensure safe and quality care, full team handing the patient, which is equivalent to the written surrender. In the event that is in oral form or in writing handing over incomplete the possibility of adverse events is greater, as risks could become a patient safety. The article is to show how important a part of the health care team and patient transfer with the complete transfer of information. The purpose of this article is to show the contents of the form that is used when transferring patients, findings or form to improve the delivery of key information. Methods: The paper used descriptive method of review and critical reading of domestic and foreign literature. The following article presents an analysis of the revised to transfer sheets of intensive care unit ward and intensive care unit operational disciplines in various departments of the General Hospital Jesenice in October, November and December of 2012. Results: The study found that important information about the health status of the patient transferred to record and transfer nurses take patient. Conclusion: The safety and quality of health care can be provided with consistent documentation and effective transfer of information on transfer of patients.
