Abstract     Theoretical background: Team approach, specialised care and continuous patient care can be achieved through the patient handoff. For the purposes of the research, we demonstrated the patient handoffs in the University Clinic of Respiratory and Allergic Diseases Golnik and introduced improvements on the basis of our findings. Methods: The research was based on the qualitative and quantitative research methodology. The research included 9 nurses employed as heads of the team at the Intensive Care Unit at the University Clinic Golnik, who are regularly involved in patient handoffs. As an instrument to collect data in the qualitative part of the research, semi-structured individual interviews with nurses working as heads of the team at the Intensive Care Unit were used. In the quantitative part, the survey questionnaire relating to the SBAR (situation, background, assessment, recommendation) report was used. The realisation of the sample in all phases was 100%. Results: On the basis of the interviews carried out we established 23 codes and 4 subcategories: factors of successful patient handoffs, factors of unsuccessful patient handoffs, development of successful patient handoffs, and causes for the development of near misses during patient handoffs. This served as the basis for the primary category: conducting successful and safe patient handoffs. Patient handoff according to the SBAR methodology enables a more transparent assessment of orally acquired information in the documentation compared to the method of free writing. Discussion: The qualitative research confirmed the deficiencies of the current patient handoff. We recognised certain opportunities for improvements and implemented them in practice. Further researches would be necessary to establish the link between patient handoffs and risks of mistake in the health care when patients are involved.Teoretična izhodišča: S predajo pacienta dosežemo timski pristop dela, specializirano oskrbo in nepretrgano zdravstveno obravnavo pacienta. Z raziskavo smo napravili posnetek stanja predaje pacientov v Univerzitetni kliniki za pljučne bolezni in alergijo Golnik (Klinika Golnik) ter na podlagi ugotovitev uvedli izboljšave. Metode: Raziskava je temeljila na uporabi kvalitativne in kvantitativne raziskovalne metodologije. V raziskavo je bilo vključenih 9 medicinskih sester na delovnem mestu vodje tima Oddelka za intenzivno terapijo in nego v Kliniki Golnik, ki se redno vključujejo v dogodek predaje pacientov. Kot instrument zbiranja podatkov v kvalitativnem delu je bil uporabljen nestandardiziran, pol strukturiran individualni intervjuju z vodji tima. V kvantitativnem delu je bil uporabljen anketni vprašalnik, ki se je nanašal na SIOP(situacija, izhodišče, ocena, priporočilo) poročilo. Realizacija vzorca v vseh fazah je bila 100 %. Rezultati: Na podlagi izvedenih intervjujev je bilo določenih 23 kod, 4 podkategorije: dejavniki uspešne predaje pacientov, dejavniki neuspešne predaje pacientov, razvijanje uspešne predaje pacientov in vzroki za nastanek skorajšnjih napak pri predaji pacientov in na podlagi tega oblikovali eno glavno kategorijo: ustvarjanje uspešne in varne predaje pacientov. Raziskava je pokazala, da proces predaje pacientov po SIOP metodi omogoča preglednejše preverjanje ustno prejetih informacij v dokumentaciji v primerjavi z metodo prostega spisa. Razprava in zaključek: S kvalitativno raziskavo smo ugotovili pomanjkljivosti trenutne predaje pacientov ter na podlagi prepoznanih priložnosti za izboljšav, le-te uvedli v prakso. Nadaljnje raziskave bi bile potrebne na področju povezave med aktivnostjo predaje pacienta in tveganjem za napako v zdravstvu pri pacientu.
