Abstract		In the last two decades an important progress in surgical treatment of the proximal humeral fractures was achived, particularly due to the angular stable - locking implants. With development of new intramedullary nails, following the same principles, an intramedullary nailing of proximal humeral fractures became a good alternative to plate osteosynthesis, with many advantages. Nevertheless, some disadvantages are hindering wider use of this otherwise excellent method.V zadnjih dveh desetletjih je prišlo do pomembnega napredka pri operativnem načinu zdravljenja zlomov začetnega dela nadlahtnice, predvsem z razvojem vsadkov po načelu kotno stabilne učvrstitve. Z razvojem intramedularnih žebljev po enakem načelu je postala tovrstna metoda učvrstitve zlomov začetnega dela nadlahtnice dobra alternativa učvrstitvi s kotno stabilno ploščo, s številnimi prednostmi. Žal nekatere pomanjkljivosti ovirajo široko razmahnitev te, sicer izvrstne metode.
