Abstract     The ideal graft for anterior cruciate ligament (ACL) reconstruction should have structural and biomechanical properties similar to the native intact ACL. Graft material for ACL reconstruction should have minimal donor site morbidity, rapid biologic incorporation and it should permit secure fixation. For graft material, several options are available: patellar tendon graft (bone-patellar tendon bone autograft, BTB), semitendinosus tendon (ST), semitendinosus and gracilis tendos (STG) and quadriceps tendon (Q). Graft choice depends on surgeon experience and preference, graft strenght, tissue availability, patient activity level, comorbidities, prior surgery, recovery time, muscle affection, the incidence of rerupture and patient preference. Although each graft material has specific advantages and disadvantages, BTB grafts are preferable for high performance athletes.
