Abstract		Starostniki so zelo heterogena skupina glede na zdravstveno in funkcijsko stanje, osebnostne različnosti ter socialno-ekonomsko stanje. Pri mnogih starostnikih opažamo polipragmazijo, ki pogosto vodi do številnih neustreznih zdravstvenih izidov. Vloga farmacevta ob izdaji zdravil starostnikom, ki so doma ali v domovih starejših občanov, je ravno zaradi polipragmzaije izredno pomembna. Farmacevti naj bi ob poznavanju meril predpisovanja zdravil starostnikom (npr. Beersova merila) prepoznali neustrezne terapije, ki lahko vodijo k pojavu neželenih učinkov. V teh primerih bi se morali o terapiji pogovoriti z zdravnikom.Elderly are a very heterogeneous group regarding health condition, stste of body functions, differences in personality and socioeconomic status. Polypragmasia is common in the elderly and often leads to many side effects and inadequate health treatment results. In such cases the role of pharmacist is very important especially if the elderly are stationed at home or at nursing homes. Pharmacists knowledge of the drug prescribing criteria in the elderly (e.g. Beers criteria) enables to recognise inappropriate therapy which could lead to adverse effects. Such cases should be discussed with a prescribing doctor.
