Abstract     A 55-year-old woman developed a severe metabolic and respiratory alaklosis accompanied by a spectrum of ventricular rhythms including torsade de pointes and ventricular fibrillation. Normal sinus rhythm was restored, without pharmacological or electrical intervention when metabolic abnormalities were corrected. On a temporal basis, correction of alkalosis may have been more important in abolishing arrhythmias than correction of hypokalaemia alone. Hypomagnesaemia might be an additional reason for torsade de pointes though this was not measured.
