Title		Glucose loading in a hypoglycaemic, shocked patient - a possible beneficial effect on haemodynamics and heart rhythm

Abstract		The effect of glucose on heart muscle has been a subject of investigation for over 80 years. The influence of a glucose infusion on myocardial contractility and electrical activity has previously been recognised. We report an observation made in a hypoglycaemic and shocked patient who exhibited atrial standstill with a junctional rhythm of 45 bpm. Treatment with positive inotropic drugs, atropine, and sodium bicarbonate gave only a minor improvement. On glucose loading, a satisfactory heart rhythm with restoration of atrial activity occurred within two minutes and haemodynamics began to improve gradually. Nevertheless, it is difficult to confirm whether glucose loading induced the observed haemodynamic improvement or was purely coincidental.
