Title     Prevention and therapy of infectious morbidity in radical gynecological surgery

Abstract     The oncological patients are at high risk for postoperative infectious morbidity. At our department all the patients submitted to radical surgical procedures received antibiotic prophylaxis (gentamycin and metronidazole) and prophylactic low dose heparin. From april 1990 to february 1992 139 patients underwent radical surgical procedure (97 cases of WERTHEIM MEIGS NOVAK-PIVER III and 23 cases of classical Wertheim-Piver II operations and 19 cases of radical vulvectomy). In radical hysterectomy patients there were 4.1 percent of pelvic infection, 3.3 percent of laparotomy wound infection and 0.8 percent of urinary tract infection. There was no case of primary mortality, but 2 cases of pneumonia and 2 cases of deep leg venous thrombosis were observed. The percentage of ureterovaginal fistula was 2.5. The age of patients, duration of operation and blood loss were not significantly related to infection. The most important factors in infection prevention, other than antibiotics, are a traumatic surgical technique, exact hemostasis, suction drainage of operative field, continuous drainage of the bladder and technically skillful surgical team.
