Title     Znanje in poznavanje uporabe avtoinjektorja adrenalina pri pacientih po anafilaktični reakciji zaradi pika žuželk

Abstract     Teoretična izhodišča: Anafilaktična reakcija po piku žuželke je življenje ogrožujoča, zdravilo izbora je adrenalin. Paciente se oskrbi z avtoinjektorjem adrenalina. Samo oskrba z avtoinjektorjem ni zadosten ukrep, pacienti potrebujejo učenje aplikacije avtoinjektorja ob vsakokratnem predpisu enkrat letno in pred sezono pojava žuželk. Pravilno ukrepanje po piku žuželke pomembno vpliva na izid zdravljenja. Metoda: Raziskovalni vzorec je zajemal 63 pacientov. Za preverjanje znanja o avtoinjektorju adrenalina je bil uporabljen anketni vprašalnikom zaprtega tipa. Uporabljena statistična metoda je bila hi kvadrat test. Za pravilno uporabo avtoinjektorja adrenalina je bil izveden eksperiment z uporabo ček liste. Za obdelavo ček liste je bil uporabljen točkovni biserialni koeficient koleracije. Raziskava je potekala od 25.1.2015 do 10.2.2015. Rezultati: Povezanost med podatkom obiskovanjem specifične imunoterapije in poznavanjem indikacij za uporabo adrenalina je statistično pomembna (%%(2) = 13,980; p < 0,01). Tisti udeleženci, ki hodijo na specifično imunoterapijo, pomembno bolje poznajo indikacije za njegovo uporabo. Pacienti, ki se zdravijo s specifično imunoterapijo, ne poznajo bolje mesta aplikacije adrenalina kot tisti udeleženci, ki se ne zdravijo s specifično imunoterapijo. Pri pacientih, ki sta jih o mehanizmu delovanja in uporabe adrenalina v injekciji seznanila zdravnik ali medicinska sestra, so prisotne bolj pravilne indikacije o pravilni uporabi adrenalina. Za oceno zanesljivosti čekliste smo izračunali Cronbachov alfa koeficient zanesljivosti, ki je 0,800. Pacienti, ki hodijo na specifično imunoterapijo, torej ne znajo pomembno bolj pravilno aplicirati avtoinjektor adrenalina. Razprava: Pacienti, ki prejemajo specifično imunoterapijo za žuželke dobro poznajo indikacije za aplikacijo avtoinjektorja adrenalina. Pomajkljivosti v znanju pa so zaznane na področju izbire mesta aplikacije in v samem načinu aplikacije tako pri pacientih na specifični imunoterapije kot pri tistih, ki ne prejemajo specifično imunoterapijo. Potreben je razmislek o ustreznem vodenju pacientov, ki se jim predpiše avtoinjektor adrenalina.Introduction: Anaphylactic reaction after an insect sting is life - threatening, the drug of choice is adrenaline. Patients are supplied with the epinephrine auto-injector. Just supplying auto-injector is not a sufficient measure, patients need learning applications auto-injector at each rule once a year and in the pre-season emergence of insects. The proper response to insect stings has a significant impact on the treatment outcome. Methods: 63 patients participated. For the examination of epinephrine auto%injector a survey questionnaire of closed type was used. The statistical methods used were chi-square test. For correct use of epinephrine auto-injector carried out an experiment using check leaves. To check processing a check leavespoint coefficient of correlation has been used. The data were collected in January and February 2015. Results: The relationship between the data attending specific immunotherapy and knowledge of the indications for the use of adrenaline was statistically significant (%% (2) = 13.980 ; p < 0.01). Those participants who were treated with specific immunotherapy have significantly better knowledge of the indications for its use. Patients, who are treated with specific immunotherapy, do not know the site application of adrenaline better as those participants who are not treated with specific immunotherapy. In patients who have them on the mechanism of action and the use of epinephrine in the injection advised by a doctor or nurse present a more accurate indication of the correct use of epinephrine. To assess the reliability of check leaves we calculated Cronbach%s alpha reliability coefficient, which is 0.800. Patients, who were treated with specific immunotherapy, cannot significantly more properly administer epinephrine auto-injector. Discussion and conclusion: Patients who receive specific immunotherapy for insects are familiar with the indications for the application of adrenaline auto-injector. Defects remain in the knowledge, they are perceived in the site selection and applications in single mode applications, such as patients to specific immunotherapy than in those not receiving specific immunotherapy. A reflection on the proper management of patients they prescribe epinephrine auto-injector. The following would be worth exploring the reasons for the shortcomings of knowledge use adrenaline auto-injector.
