Abstract | Pomanjkanje vitamina D je postalo pandemija. Najpogostejši vzrok zanjo je zmanjšanje zmerne izpostavljenosti soncu, ki je glavni vir vitamina D. Le redka hrana naravno vsebuje vitamin D in tudi tista z dodanim vitaminom D ga ima pogosto premalo, da bi zadostila potrebam otroka ali odraslega. Pomanjkanje vitamina D pri otrocih povzroča rahitis, pri odraslih pa osteopenijo, osteoporozo in zlome. Osteoporotični zlomi vretenc niso edini vzrok za bolečino v hrbtu, saj nekatere študije kažejo, da za pojav bolečine vkrižu lahko zadošča že samo pomanjkanje vitamina D. Priporočena plazemska vrednost 25-hidroksivitamina D je 75 nmol/L (30ng/mL). Ob odsotnosti ustrezne izpostavljenosti sončni svetlobi svetujejo nadomeščanje z 800-1000 IE vitamina D dnevno tako pri otrocih kot pri odraslih. Posebno pozornost pri nadomeščanju vitamina D moramo posvetiti tistim bolnikom, ki se morajo zaradi kožnega obolenja strogo izogibati soncu.Vitamin D deficiency is now recognized as a pandemic. The major cause of vitamin D deficiency is lack of moderate sun exposure, which is the major source of vitamin D. Very few foods naturally contain vitamin D, and foods that are fortified with vitamin D are often inadequate to satisfy either a childs or an adults vitamin D requirement. Vitamin D deficiency causes rickets in children and will precipitate osteopenia, osteoporosis, and fractures in adults. Osteoporotic vertebral fractures are not the only cause of back pain as some studies indicate that low back pain can be caused by vitamin D deficiency alone. A circulating level of 25-hydroxyvitamin D of 75 nmol/L, or 30 ng/mL, is required to maximize vitamin D's beneficial effects for health. In the absence of adequate sun exposure, at least 8001000 IU vitamin D/day may be needed to achieve this in children and adults. Special attention to appropriate substitution of vitamin D should be given to those patients who should strictly avoid sun exposure due to their skin conditions. | |
