Abstract | Background: Infective endocarditis (IE) although rare is a disease that contributes significantly to mortality and morbiditiy of patients with valvular disease. So far no clinical data on patients with IE and prognostic factors influencing the outcome of IE have been published in Slovenia. Aim of study: The aim of the study was to describe epidemiological, microbiological and clinical characteristics of our patients, to evaluate our clinical, echocardiographic and other methods for the diagnosis and treatment of IE and to asses prognostic factors, influencing the immediate as well as long term outcome of the disease. Material and methods: In the years 1984 to 1996 all the patients with the diagnosis of IE hospitalized at the Department of Infectious Diseases in Ljubljana were observed. The patients were followed in the outpatient clinic to the end of the year 1997. Patients were included in the study according to the criteria of Duke Endocarditis Service. Data recorded for each patient were: age, sex, previous valvular disease, localization and type of previous heart disease, possible entrance site of theinfection, presenting symptoms and clinical signs, duration of hospitalization, duration of symptoms and temperature, frequency of typical clinical signs and symptoms, timing of complications, laboratory parameters (SR, CRP, fibrinogen, gammaglobulins, LDH, hemoglobin and erythrocye count), isolated pathogen, echocardiographic changes and type of treatment. After the hospitalization the patients were regularly followed. End-points of observation were relapse, recurrence, heart surgery and death. The data were statistically processed using Epi Info 6 program. Statistically significant variables were then entered in Cox regression model. Results: Two-hundred and five episodes of IE fulfilled the Duke criteria for definite or possible endocarditis. (Abstract truncated at 2000 characters.) | |
