Abstract		Background. A case report of a 76-years old patient with a fresh, total rupture of the Achilles tendon is reported. The sub(per)cutaneous reconstruction of the ruptured Achilles tendon proceeding our method under local anaesthesia was carried out after the clinical and ultrasonographical confirmation of the rupture. Conclusions. Debate concerning the appropriate treatment of fresh, total ruptures of the Achilles tendon still continues. Nevertheless most authors prefer open surgical repair. Percutaneous reconstruction combines the advantages of the conservative and open surgical treatment and reduces risk of complications as well as costs. Operating with local anaesthesia we can avoid the complications and risk of general or spinal anaesthesia, there are almost no contraindications for the intervention.
Summary		Izhodišča. Prikazan je primer 76-letne bolnice s svežim popolnim pretrganjem Ahilove tetive. Po klinični in ultrazvočni potrditvi je bila operirana po naši metodi sub(per)kutanega šiva v lokalni anesteziji. Zaključki. Mnenja o ustreznosti konservativnega oziroma operativnega načina zdravljenja svežih popolnih pretrganj Ahilove tetive so še vedno različna. Kljub vsemu prevladuje odprta operativna oskrba svežih poškodb Ahilove tetive. Perkutana metoda združuje prednosti konservativne metode in odprte operativne oskrbe, s čimer pomembno zmanjšamo možnosti komplikacij in tudi stroške. Z operiranjem v lokalni anesteziji se izognemo nevarnostim splošne oziroma spinalne anestezije, za metodo pa praktično ni kontraindikacij.
