Abstract		Background. Operative procedure in Achilles tendon rupture was significantly simplified by subcutaneous sutures method. The duration of hospitalization was shortened. This method was successfully applied to operative treatment of avulsion fracture of the tendon's insertion on the calcaneus. Results. From 1985 to 1994 three patients with avulsion of the Achilles tendon insertion on the calcaneus were treated operatively. Twice lag screws and once tension band were applied. Due to patients age the bone density was significantly reduced and osteosynthetic matherial failed to retain the bony fragment. Redislocation occurred. At reoperation the avulsion fracture was treated as the Achilles tendon rupture at its lower end. With subcutaneous suture the tendon was fixed to the calcaneus. Conclusions. With subcutaneous sutures the better aesthetic outcome is achieved with lesser postoperative complications. There is no need for operative removal of screws and wires and the duration of hospitalization is shortened.
Summary		Izhodišča. S subkutanim šivanjem raztrganine Ahilove tetive so bistveno poenostavili operativni postopek in skrajšali hospitalizacijo. Metodo so uporabili tudi pri odtrganini narastišča tetive na petnico. Rezultati. Od leta 1985-1994 so operirali tri bolnike s tako poškodbo. Dvakrat so uporabili pritezni vijak, enkrat žično zanko. Zaradi starosti poškodovancev je bila kostna gostota pomembno zmanjšana. Ob vijaku se je kostna luska zdrobila in ponovno dislocirala. Pri reoperaciji so ravnali, kot da je raztrgana tetiva na spodnjem koncu. S subkutanim šivom so jo prišili na petnico in krhko kostno lusko obšli. Zaključki. Poseg s subkutanim šivom je za poškodovanca manj zahteven. Pooperativni potek je lažji in estetski rezultat boljši. Zaradi uporabe resorbilnega šivalnega materiala ni potrebno kasnejše operativno odstranjevanje vijakov.
