Abstract     This study is based of 60 human cadavers in both sexes. We have found that in most of the cases (75.40%) the sympathetic trunk (TS) passes behind the muscular part of the medial diaphragmatic crus (MCM), rather rarely (19.67%) it passes lateraly to the MCM, and very rarely (4.91%) it passes through the MCM itself. In most of the cases (67.20%) the TS passes between the main tendon of the medial diaphragmatic crus (TCM) and the tendon of its intermediate crus (TCI). Very rarely (4.92%) TS passes between TCI and the psoas muscle (MPM) and in the cases where the intermediate crus is absent, TS passes between TCM and MPM (27.87%). In all the cases examined, the structures between which the TS passes are always connected by a fibrous arch: its lower margin is projected, in nearly half of the cases (45.90%), on the intervertebral disc between L1 and L2. In the rest of cases, the lower margin of the fibrous arch corresponds more often (31.14%) to L2 than L1 (22.95%). The upper margin of this arch does not exist because in fibers interfere with the fibers of dascia transversalis that covens the muscular part of the medial diaphragmatic crus.
