Abstract     Myofascial pain syndrome is one of the most important chronic problems encountered in a clinical practice and is defined as regional pain syndrome, characterized by the presence of myofascial trigger points (TrPs). But still, among clinicians opinions differ a lot regarding many aspects of myofascial pain. In this article two problems are in focus: the problem of reliability of clinical diagnosis and the possibilities of treatment. The clinical evidence for and against the common therapeutic interventions is reviewed and a pilot study of pain intensity and quality of life improvement of patients with cervicobrachialgia treated by dry needling in our institution is presented. The possibilities how to improve the management of this complex problem are discussed at the end.
