Abstract     Izhodišča: V otroštvu se težave, povezane s hranjenjem, pojavljajo relativno pogosto, še posebej pri otrocih z nevrološkimi motnjami. Učinkovito in varno hranjenje v zgodnjem obdobju neposredno vpliva na rast in razvoj otrok. Zanesljiva klinična ocena hranjenja in požiranja omogoča zgodnjo prepoznavo motenj hranjenja in požiranja ter predstavlja izhodišče za načrtovanje pomoči pri vzpostavljanju učinkovitega hranjenja. V praksi za ocenjevanje hranjenja in požiranja uporabljamo presejalni test za oceno motenj hranjenja in požiranja (angl. Dysphagia Disorder Survey, DDS). Za oceno je potrebno snemanje hranjenja. V študiji smo želeli preveriti, če je neposredna ocena hranjenja s testom DDS (v živo) enako zanesljiva kot ocenjevanje posnetka hranjenja. Metode: S testom DDS smo ocenjevali hranjenje in požiranje pri otrocih z nevrološkimi motnjami. Posneli smo hranjenje posameznega otroka, hkrati pa je med snemanjem ena od logopedinj neposredno ocenila hranjenje. Posnetek hranjenja je nato v oceno dobila druga logopedinja. Na neposreden in posreden način pridobljene ocene smo nato primerjali med seboj. Za prikaz skladnosti obeh ocen hranjenja s testom DDS smo uporabili intraklasni korelacijski koeficient. Rezultati: Ocenili smo 19 otrok v starosti od tri do 12 let. Statistična analiza podatkov je pokazala visoko skladnost pri skupnem dosežku ocenjevanja hranjenja s testom DDS v živo in retrogradno s pomočjo posnetka (ICC 0,98) in pri večini posameznih postavk testa (ICC med 0,68 in 0,96). Zaključek: Rezultati študije so pokazali, da je ocenjevanje hranjenja s testom DDS za oceno motenj hranjenja in požiranja v živo in s pomočjo posnetka visoko zanesljivo, zato lahko ocenjevanje teče tudi brez snemanja hranjenja.Introduction: Feeding and swallowing disorders are relatively frequent in early childhood, especially in children with various neurological impairments. Effective and safe feeding in the early childhood directly affects the growth and development of children. Reliable clinical assessment of feeding and swallowing represents a starting point for the effective management and therapeutic treatment of children. The study aimed to evaluate whether the direct assessment in vivo is as reliable as video-recorded assessment of feeding with Dysphagia Disorder Survey (DDS). Methods: With the DDS test we evaluated feeding and swallowing of children with neurological impairment. Children were video-recorded during feeding or eating a usual meal. The first speech pathologist assessed children eating in vivo, while the recording was being prepared. The second speech pathologist assessed those recordings later. In order to demonstrate the agreement between in-vivo and video-recorded evaluation, we calculated the intra-class correlation coefficient. Results: We evaluated 19 children with developmental neurological impairment aged from three to 12 years. Statistical analysis revealed a high intra-class correlation coefficient of the total DDS score (ICC 0,98), as well as for the majority of DDS items (ICC between 0,68 and 0,69). Conclusions: The assessment of eating with DDS in vivo and based on video- recordings is highly reliable. Based on that we believe that the DDS could also be used without previous video-recording of eating.
