Abstract     The number of patients with hilar cholangiocarcinoma is increasing world wide. The sophisticated diagnostic methods enable us to find accurate diagnosis preoperatively, to assess the resectability of the tumour and select the patients who are suitable for radical resection. Aggressive surgical treatment gives promising results and palliative procedures have been reserved for unresectable tumours. From January 1987 to December 1993 46 patients with Klatskin tumour were operated at our department. The resectability rate was high (73,9 percent); 34 resections of common bile duct were performed. in 50 percent of patients hepatectomy was additionally accomplished. Tumours were classified according to Bismuth- Corlette staging. At 12 patients various types of palliative procedures were performed. The authors discuss the need for good preoperative management of patients with obstructive jaundice, indications for radical versus palliative procedures and the need for aggressive surgical treatment in suitable patients.
