Abstract     An investigation of smoking and psychosocial factors in a group of patients with ischemic heart disease and in a control group has been launched in order to establish significant risk factors and to verify the existence of possible relations (correlations and synergistic interactiones) between smoking and significant psychosocial risk factors for the ischemic heart disease. Nearly two hundred patients with ischemic heart disease (diagnosis based on coronarography) were compared with the same number of hospitalized control cases matched by sex and age (ischemic heart disease was excluded by anamnestic data and careful survey of up to the present medical documentation and if necessary also by clinical check-up and specific diagnostic methods). Data were collected regarding smoking habits (whenever in life), the number of smoked cigarettes and the following psychosocial factors: basic personality traits, coronary behavioural type and some other psychosocial types and different ways of coping with stress. The results show that smokers are at greater risk for ischemic heart disease as non-smokers (p less th. 0.001); but the risk does not differe regarding the number of smoked cigarettes. Emotional unstability is the most important factor for the ischemic heart disease (p less th. 0.001). Smokers are characterized by the expected psychosocial characteristics: emotionally unstable personality and lack of personal autonomy; the last being still more characteristic for smokers with more smoked cigarettes (in comparison with those who smoke less). The number of smoked cigarettes is related also to the "disease prone" behaviour and to the non-adapted patterns of stress control. It is possible to conclude that smoking does present a risk factor for ischemic heart disease, but smoker swith this disease smoke the same amount of cigarettes as smokers without it.(trunc.)
