Abstract		Primary health care in the counties of Istria and Primorje-Gorski kotar has been developing evenly in all the localities. Health Centres which differ according to the size and organizational forms of activities, are planning, organizing and implementing primary specific health care as well as a part of public health care on the domain of their activity. By the regulations based on the Law on health institutions, health care and health insurance, the possibility of their transformation is ensured. Transformation of health Centres is necessary for several reasons, among which we stress large Health Centres in urban areas which had become bureaucratized and could deteriorate. However, in the health Centres transformation proceedings, it is necessary to consider a graduate and scientifically based approach to avoid severe consequences in health care implementation. Health Centres are our tradition, therefore their certain functions should be retained in the interest of the entire health care.
