Abstract     Background. In intensive care units and other units of our hospitals where neutropenic and immunocompromised patients are treated we observe the appearance of methicillin-resistant S. aureus. Methicillin-resistant S. aureus is responsible for serious infections in immunocompromised patients. The mechanism of resistance to methicillin is alternation of penicillin-binding proteins from PBP to PBP 2a. Methicillin-resistant staphylococci are resistant also to other beta-lactam antibiotics. Methods. Laboratory detection of methicillin-resistant S. aureus is based on agar dilution method and microdilution and agar dilution methods. The proper recognition of methicillin-resistant S. aureus is necessary to prevent spreading of resistant staphylococci and to treat patients with infections caused by resistant staplylococci. Conclusions. The sources of resistant S. aureus in hospitals are patients and staff who spread bacteria via their hands. Inanimate hospital enviroment is less important source of staphyloccoci. Patients and staff colonized with methicillin-resistant S. aureus should be isolated in contact isolation and treated with topic antibiotics. Neutropenic patients should receive systemic prophylaxis when colonized. Patients with infections should be treated with vancomycin alone or in combination with aminoglycosides or rifampicin. Careful hand washing and disinfection, cleaning and disinfection of surfaces in units with immunocompromised patietns and other activities will help to prevent spreding of methicillin-resistant S. aureus to other patients.
Summary     Izhodišča. V intenzivnih in drugih enotah naših bolnišnic, kjer zdravimo bilnike z nevtropenijo in pomanjkljivim imunskim odzivom, opažamo pojavljanje Staphylococcus aureusa (S. aureusa), ki je odporen proti protistafilokoknim penicilinom in Številnim drugih antibiotikom. Povzroča hude okužbe pri imunsko oslabljenih bolnikih. Odpornost stafilokokov proti meticilinu je posledica spremenjenih beljakovin za vezavo penicilinov (PBP) v celični steni bakterij. Beljakovine za vezano penicilinov PBP v celični steni se pri njih spremenijo v PBP 2a. Zaradi spremembe beljakovin so odporni tudi proti drugim beta-laktamskim antibiotikom. Metode. odpornost stafilokokov proti meticilinu lahko ugotavljamo z metodo difuzije v agarju z diski in z metodami razredčitve antibiotika v bujonu in v agarju. Pravilno prepoznavanje odpornega S. aureusa v laboratoriju je pomembno zato, da preprečimo njegovo širjenje in da bolnike z okužbami pravočasno zdravimo z ustreznimi antibiotiki. Zaključki. Vir odpornega S. aureusa v bolnišnici so predvsem bolniki in osebje, ki razširjajo te bakterije z rokami. Manj pomembni so predmeti in prah v okolju. Bolnike in osebje, ki so z odpornim S. aureusom le kolonizirani, osamimo in zdravimo z antiseptičnim mazilom. Imunsko oslabljenim bolnikom dajemo antibiotično profilakso. Bolnike z okužbo zdravimo z vankomicinom, lahko v kombinaciji z aminoglikozidom ali rifampcinom. Prenos odpornega S. aureusa na druge bolnike preprečujemo tudi z umivanjem in razkuževanjem rok ter s čiščenjem in razkuževanjem prostorov.
