Title     Stopnja znanja o Alzheimerjevi bolezni, starosti in odnos do dela s starostniki med zaposlenimi v domovih za starejše v Sloveniji

Abstract     Teoretična izhodišča: Pomemben javnozdravstveni problem razvitega sveta predstavljajo kronične bolezni, med njimi še posebej demenca. Tekom svoje kariere so skoraj vsi zdravstveni delavci do neke mere vključeni v zdravstveno obravnavo starostnika. Kakovost oskrbe starih ljudi pa je neposredno povezana z znanji in odnosom zdravstvenega osebja do njih. Metoda: Uporabili smo kvantitativno eksplorativno raziskovalno metodo na priložnostnem vzorcu 242-ih izvajalcev zdravstvene nege v 13-ih domovih za starejše v Sloveniji. Uporabili smo preverjene in validirane merske instrumente (FAQ-R, POWP, ASD, ADKS). Raziskava je bila izvedena 2017. Rezultati: Povprečno število doseženih točk na testu FAQ-R znaša 23,47 (SO = 3,76), kar predstavlja 65,2% uspešnost na testu. Z višjo stopnjo izobrazbe s področja zdravstvene nege (p < 0,001) in z višjim položajem na delovnem mestu (p = 0,003) se poveča uspešnost na testu FAQR. Anketiranci so na POWP instrumentu dosegli 78,4% možnih točk, iz česar lahko sklepamo, da je med anketiranci odnos do dela s starostniki pozitiven. Negativen odnos do starostnikov ugotavljamo pri 7,8% anketirancev, pozitiven pri 45,2% in nevtralen pri 46,9% anketirancev. Povprečno število doseženih točk na ADKS testu znaša 19,47 (64,9% možnih točk). Ugotavljamo statistično pomembne razlike v doseženih točkah glede na izobrazbo na področju zdravstvene nege (p < 0,001); najnižje število točk so dosegli anketiranci z dokončano srednjo poklicno šolo, najvišje pa anketiranci z dokončano višjo šolo ali več. Povezanost med dobljenimi točkami na testu ADKS in starostjo je nizka in pozitivna (r = 0,150) ter statistično značilna (p = 0,022). Razprava: Raziskava kaže, da je stopnja znanja o Alzheimerjevi bolezni med izvajalci zdravstvene nege v zdravstvenih domovih zadovoljiva, a da se slabša z nižanjem izobrazbe. Splošno znanje o starostnikih, staranju in duševnih boleznih v starosti med njimi je pomanjkljivo. Ugotavljamo, da se njihov odnos do starostnikov giblje med pozitivnim in nevtralnim. Odnos do dela z njimi se nagiba v pozitivno smer. Naši rezultati so mednarodno primerljivi. Raziskavo bi bilo potrebno razširiti na vse domove starostnikov v Sloveniji ter tudi na izvajalce zdravstvene nege na primarnem, sekundarnem in terciarnem nivoju zdravstva.Background: Chronic diseases, especially dementia, represent a major public health concern in developed countries. Over the course of their careers, almost all health care workers participate in health care provision to the elderly, at least to some extent. Importantly, the quality of care given to elderly people directly correlates with the knowledge and attitudes of health care workers towards this age group. Methods: Quantitative exploratory research design was employed on a convenience sample of 242 nursing care providers in 13 Slovenian nursing homes. Tried and validated data collection instruments were used (FAQ-R, POWP, ASD, ADKS). The research was conducted in 2017. Results: The mean score obtained on the FAQ-R test was 23.47 (SD = 3.76), which translates into an accuracy of 65.2%. Respondents with a higher educational achievement in nursing (p < 0.001) and a higher position at work (p = 0.003) achieved higher accuracy on the FAQ-R test. On the POWP instrument, the average score was 78.4%, indicating a positive attitude towards work with the elderly. Negative and positive attitudes towards the elderly were established in 7.8% and 45.2% of respondents, respectively, while 46.9% had a neutral attitude. The mean score obtained on the ADKS test was 19.47 (64.9% of total points). Statistically significant differences were established for the obtained scores according to educational achievement in nursing (p < 0.001)% respondents with vocational secondary school had the lowest scores, compared to respondents with a junior college or higher who had the highest scores. The correlation between the obtained ADKS scores and respondent age was low and positive (r = 0.150) and statistically significant (p = 0.022). Discussion: Research results revealed that the level of knowledge on Alzheimer's disease among nursing care providers in Slovenian nursing homes is satisfactory; however, it decreases with a lower education. The general knowledge on the elderly, ageing, and mental disorders in old age among nursing care providers is inadequate, and their attitudes towards the elderly range from positive to neutral while their attitude towards work with the elderly leans towards the positive. The obtained results are internationally comparable. The research would have to be broadened to include all Slovenian nursing homes and nursing care providers at the primary, secondary, and tertiary levels of health care.
