Abstract | Izhodišča: Amiotrofična lateralna skleroza (ALS) je nevrodegenerativna bolezen z napredujočo mišično oslabelostjo. Ob napredovanju bolezni se razvije šibkost dihalnih mišic, ki vodi v zmanjšano predihanost, zaradi česar bolniki potrebujejo dihalno podporo. Pri bolnikih z ALS najpogosteje uvedemo neinvazivno ventilacijo (NIV) s podporo dihanju na dveh tlačnih ravneh (IPAP, EPAP), s čimer izboljšamo kakovost in dolžino preživetja. Metode: V prispevku so opisane težave z dihanjem in meritve, ki jih pri bolnikih z ALS opravljamo rutinsko, vključno s funkcionalno lestvico ALSFRS-r. Predstavljena je neinvazivna ventilacija pri bolnikih z ALS. Rezultati: Prikazani so primeri različne uporabe NIV: intermitentna uporaba, uporaba preko noči, stalna uporaba NIV. Predstavili smo tudi uporabnost razbiranja informacije s SD kartice ventilatorja in grafično prikazali rezultate uporabe aparata za NIV. Zaključki: Pri bolnikih z ALS je potrebno posebno skrb nameniti funkcijam dihanja in dihalni podpori. S poznavanjem možnosti, ki jih nudijo sodobne aparature, vključno z natančnim in sprotnim spremljanjem funkcij dihanja in uporabo dihalnega aparata, lahko zagotovimo dobro oskrbo bolnikov z ALS na domu. Kakovost in preživetje pri bolnikih, ki se uspešno privadijo na uporabo NIV, se znatno izboljša.Backgrounds: Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disorder characterized by progressive decrease in muscle strength, including respiratory muscle weakness, leading to a reduced alveolar ventilation and an increasing demand on respiratory support. Frequently, in patients with ALS, who have respiratory insufficiency, we introduce non-invasive respiratory support - BiPAP mode, which prolongs survival and improves quality of life. Methods: We describe respiratory problems, and measurements we perform routinely in patients with ALS, ALSFRS-r functional rating scale included. Noninvasive ventilation is presented. Results: In selected patients we show different patterns of respiratory support: intermittent; during the night; and continuous NIV. We show the importance of reading from SD card for a comprehensive intervention. Graphically we present results of ventilators's use. Conclusions: In ALS patients special attention should be paid to respiratory function. Knowing the options which are offered in ventilation machinery used, we can promptly and accurately meet the patients's needs and enhance the home use of NIV, hereby prolonging the survival and improving quality of life in patients with ALS who have adapted to NIV easily. | |
