Abstract     In several test model systems, using spontaneous metastasing experimental tumors, convincing data indicate the importance of the tumor burden, left after surgery, for the efficacy of the combination of surgery and chemotherapy. Early removal of the primary tumor by radical surgery for cure seems to improve the conditions for chemotherapy. Since 1979 in 9 different departments of thoracic surgery patients with SCLC are randomized after surgery for cure to recive a new sequential intermittent polychemotherapy (sq. CT) of 3 different alternating drug combinations given intermittently within 1 year or one 4-drug combination-chemotherapy (CT) given intermittently within 3 years after operation. The calculation of their lifetable curves per August 1st, 1984, indicate the improvement of the 4-year survival rate of 23 patients receiving sq. CT about 50% compared with the survival rate of about 30% of 29 patients receiving CT. For firm conclusions the number of patients is still too small but we intend to conclude that surgery for SCLC seems to be an advisable measurement for the efficacy of agressive intermittent long term polychemotherapies. However this can only be proven in large cooperative studies. Therefore we prepare a new protocol together with an enlarged group accussion at the 13th International Congress of Chemotherapy (ISC) at Vienna in 1983.
