Abstract     Izhodišča: V več raziskavah je bilo potrjeno, da imajo otroci in mladostniki s cerebralno paralizo (CP) zmanjšano aerobno zmogljivost v primerjavi z zdravimi vrstniki. Znano je tudi, da vadba hoje na robotski napravi izboljša shemo hoje, zmanjša spastičnost in izboljša splošno zmogljivost. Namen študije je bilo preveriti, ali pri otrocih s cerebralno paralizo po vadbi hoje na robotski napravi Lokomat pride do izboljšanja aerobne zmogljivosti. Metode: V študijo smo vključili 17 otrok in mladostnikov; 16 s CP in eno dekle s hereditarno spastično paraparezo. Glede na grobe zmožnosti gibanja so bili otroci s CP razvrščeni v stopnje od I. do IV. Opravili smo obremenitveno testiranje po standardnem zveznem protokolu pred in po končani vadbi hoje na Lokomatu (od 16 do 18 vadbenih enot, enkrat na dan, brez dodatne fizioterapije, v obdobju štirih tednov). Rezultati: Maksimalni krvni tlak po zaključeni vadbi na Lokomatu je bil ob obremenitvi nižji, vendar razlika ni bila statistično značilna. Ob tem smo opazili tako absolutno kot relativno izboljšanje pri vseh ostalih opazovanih spremenljivkah, vendar tudi tu razlike niso bile statistično pomembne. Ob zaključku vadbe na Lokomatu so imeli preiskovanci relativno višjo maksimalno porabo kisika. Ob tem je prišlo do relativnega padca maksimalnega sistoličnega tlaka in relativnega povečanja srčne frekvence. Zaključek: Aerobna zmogljivost vključenih otrok in mladostnikov je v primerjavi z zdravimi vrstniki pomembno zmanjšana. Z vadbo hoje na Lokomatu je aerobno zmogljivost pri otrocih in mladostnikih s cerebralno paralizo mogoče izboljšati, vendar je za dokončne zaključke v raziskavo potrebno vključiti več preiskovancev.Background: Recent studies showed that aerobic capacity of children and adolescents with cerebral palsy (CP) is significantly lower compared to healthy peers. The assisted robotic gait training programs are reported to lower spasticity, improve gait pattern and general physical capacity. The aim of our study was to evaluate ton what extent could the assisted-robotic gait training improve the aerobic capacity. Methods: We included 17 children and adolescents; 16 with CP and one with hereditary spastic paraparesis. Children with CP were classified based on their gross motor function. The bicycle stress testing was performed by continuous protocol. Children were referred into assisted-robotic gait training program (16 to 18 sessions, once a day, without additional physiotherapy program, in the period of four weeks): At the end of training, the stress testing was repeated. Results: The maximal blood pressure was lower after the end of training period, but the difference was not statistically significant. We also noticed relative and absolute improvement of other variables, but none of those was statistically significant either. Participants showed relatively higher oxygen consumption, with relative drop of maximal systolic pressure and relative increase of heart frequency. Conclusions: Aerobic capacity of included children and teenagers was significantly lower than in healthy peers. Assisted-robotic gait training on Lokomat device could improve aerobic capacity, but we should include a larger group of participants to reach more reliable conclusions.
