Abstract     The intravenous route is the preferred route administration of most emergency drugs in CPR and other medical emergencies. However, periferal IV access is often difficult or impossible to obtain. In such cases endotracheal intubation turns out to be an easier and quicker procedure providing also an alternative route for drug administration before a vascular access can bestablished. Epinephrine, atropine, lidocaine, naloxon, bretylium and probably diazepam can be administered trouhg an endotracheal tube. Several studies demonstrated that depth an method of instillation do not seem to be important factors regarding delivery of medications to apsorptive sites in the lung. The dose should be 2.5 times the IV dose. The drug should be diluted in normal saline to 10 ml. Sterile water as a diluent has too much of a detrimental efffect on blood gases. When diluted drug is ready in a syringe, needle should be removed and external cardiac massage temporarily interrupted so that the drug can be rapidly injected into outer aperture of the endotracheal tube. 4 - 5 forced manual hyperventilations are the essential to assure optimally bilateral and distal delivery of endotracheally administered medications.
