Abstract     Izhodišče: V pilotni raziskavi smo želeli preveriti ustreznost protokola aerobne vadbe s pomočjo ročnega kolesa pri bolnikih po amputaciji spodnjega uda zaradi periferne arterijske bolezni (PAB). Zanimalo nas je, ali dodatna individualno predpisana 4-tedenska aerobna vadba izboljša telesno zmogljivost v večji meri kot standardni program bolnišnične rehabilitacije za bolnike po amputaciji. Ob povečanju aerobne zmogljivosti smo ob zaključku rehabilitacije pričakovali večjo vztrajnost in hitrost hoje s protezo. Metode: V raziskavo smo vključili 20 bolnikov, ki so bili prvič sprejeti na rehabilitacijo po amputaciji spodnjega uda v Univerzitetni rehabilitacijski inštitut Republike Slovenije - Soča. Bolnike smo naključno uvrstili v vadbeno in kontrolno skupino. Obe skupini sta bili vključeni v redne programe rehabilitacije, v okviru katerih poteka tudi vadba s pomočjo ročnega kolesa. Bolniki v vadbeni skupini so trikrat tedensko, štiri tedne izvajali vadbo s pomočjo ročnega kolesa z intenziteto med 70 % in 80 % rezerve srčnega utripa, ki smo ga določili na podlagi obremenitvenega testiranja. V kontrolni skupini število vadb, čas trajanja in intenziteta vadbe niso bili določeni. Bolniki so vadili glede na subjektivni občutek trenutne zmogljivosti. Rezultati: Ob zaključku 4-tedenskega programa je bila v vadbeni skupini najvišja poraba kisika (VO2peak) pri obremenitvenem testiranju statistično značilno višja. Prehojena razdalja pri 6-minutnem testu je bila pomembno daljša le v vadbeni skupini bolnikov po podkolenski amputaciji. Vsi bolniki so 4-tedensko vadbo zmogli v celoti, brez zapletov. Zaključek: Glede na rezultate pilotne študije sklepamo, da bi individualno predpisan aerobni trening pri izbranih bolnikih lahko prispeval k večji telesni zmogljivosti in učinkovitejši hoji s protezo po amputaciji spodnjega uda zaradi PAB.Background: In a pilot study, we wanted to verify the adequacy of the handwheeled aerobic exercise protocol in patients after lower limb amputation due to peripheral arterial disease (PAD). We wanted to find out whether an additional individually prescribed fourweek aerobic exercise would improve physical performance to a greater extent than the standard inpatient rehabilitation program for patients after amputation. With the increase in aerobic capacity, we expected better aerobic capacity and speed of walking with the prosthesis at the end of rehabilitation. Methods: The study included 20 patients who were admitted to rehabilitation for the first time after lower limb amputation at the University Rehabilitation Institute, Republic of Slovenia. The patients were randomly assigned to the exercise and control group. Both groups were included in the regular rehabilitation programs, which include exercise with the help of a hand wheel. The patients in the exercise group exercised three times a week for four weeks using a hand wheel with an intensity between 70 % and 80 % of the heart rate reserve, which was determined using cycle ergometer. In the control group, the number of exercises, duration and intensity of exercise were not determined. The patients exercised according to a subjective sense of current performance. Results: At the end of the four-week program, the highest oxygen consumption (VO2peak) in the exercise group was statistically significantly higher in the exercise group. The distance walked in the 6-minute test was significantly longer only in the exercise group of patients after below-knee amputation. All the patients were able to exercise for 4 weeks completely, without complications. Conclusion: Based on the results of the pilot study, we conclude that individually prescribed aerobic training in selected patients could contribute to greater physical performance and more effective walking with a prosthesis after amputation of the lower limb due to PAD.
