Abstract     Neuroblastoma is a malignant tumor of the sympathetic nervous system, representing about 5 percent of all childhood malignancies. The survival of neuroblastoma patients remains poor in spite of some improvement in treatment. A retrospective clinical analysis has been performed of neuroblastoma patients, treated in two periods of time, 1970-1979 and 1980-1993. These periods were chosen because several improvements in the diagnostic methods as well as treatment have been introduced after 1980. Sonography, computerized tomography, I-31 metaoidobenzylguanidine (MIBG) isotope scanning in diagnostics and new cytotoxic agents, new chemotherapeutic protocols, treatment with I-131 MIBG and bone marrow transplantation have been introduced in therapy. The question was whether, due to these improvements, there has been any improvement in the survival of neuroblastoma patients after 1980. Seventy-eight neuroblastoma patients, treated at the Department of Pediatrics and at the Institute in Oncology in Ljubljana 1970-1993 were included in the study. The observation period was from first admission for neuroblastoma until last follow-up or until death. The follow-up time was 0.06-24.7 years, median 1.6 years. A univariate statistical analysis has been performed of the time period as well as the prognostic factors: stage, age at diagnosis, sex, primary site and tumor size, VMA and catecholamine concentration in urine at diagnosis. The factors, which in univariate statistical analysis were found to influence survival significantly, were the, together with the period of time, included into a multivariate survival analysis. A univariate analysis has also been performed of the influence of the primary treatment response and of the extent of surgery on the survival. In the multivariate analysis the period of time (relative risk (Rr)=0.43, p=0.19), the size (Rr=3.44,p=0.06) and site of primary tumor (Rr=1.3-3.5,p=0.013), the stage of disease (Rr=2.75-6.95,p=0.(trunc.)
