Abstract     Background. Phacoemulsification at high vaccum levels and programmable rise time offers advatages like shorter ultrasound time, lower ultrasoundpower, greater holding ability, better followability, improved cutting efficiency and easier phaco-aspiration of dense cataracts. Methods. Through paracentesis incision connected to a pressure transducer IOP has been monitored during operation. Phaco machine with enhanced automatic surge prevention, cobra Phaco tip in combination with phaco chop technique has been used. Results. Hydrodynamics in relation to vacuum, flow and IOP is discussed and best settings on our machine for this surgical technique were established. Conclusions: Vacuum setting between 250 and 350 mmHg and flow rate of 25 ccm enables us effective and safe phaco cataract surgery.
