Abstract     Background. First generation cephalosporins were evaluated as empirical treatment of community-acquired pneumonia and compared to other empiricaly chosen antibiotics. Methods. In reaprospective study, patients over 15 years of age, hospitalised in year 1988 and 1993, treated for community-acquired pneumonia were included. Age of patients, month of admission, emprically chosen antimicrobials, underlying conditions and treatment outcome of the patients were recorded. Statistical analysis was performed using the chi-square test. Results. Empirical antimicrobial treatment of communitiy-acquired pneumonia with first generation cephalosporins was successful in 70 percent of cases in year 1988 and 74 percent five years later. The success rate was not statistically significant different comparing the group of patients treated with cephalotin or cefazoline and the group treated with other empirically chosen antimicrobials. Conclusions. First generation cephalosporins are still recommended as possible empirical treatment of community-acquired pneumonia for older patients or patients with preexisting diseases.
