Abstract     From May 1983 to April 1989 51 patients from 25 to 79 years old were treated with CAPD. During this time net ultrafiltration (NUF), changes of intraperitoneal volume, residual volume and peritoneal clearances for urea (PKU), creatubube (PKK) abd veta-2-microglobuline were studied when 1.36 (GDR 1.36) or 3.86% (GDR 3.86) glucose dialysis fluids were used. Measurements were made during the course of peritonitis and once in the year. During CAPD catheter insertion or removal a b iopsy of parietal peritoneum was made, specimens were prepared for light (LM) and transmission electron microscopy (TEM). During this time NUF and clearances were unchanged. In the course of severe peritonitis NUF was smaller, PKU higher (P less.th 0,05) if 3.86 GDR was used. Despite the high frequency of peritonitis the function of peritoneum remained unchanged. LM changes during inflammation were: distruction of mesothelial cells, edema and multiplication of submesothelial collagen fibers, congestion of vessels, infiltration with plasma cells, granulocytes, and hemorrhages. With TEM the most impressive finding was splitting of basal membrane of small vessels. But, from our observations we can conclude, that the function of peritoneum remained stable during the long-term CAPD despite the high frequency of peritonitis. During peritonitis histological changes coincided with functional changes.
