Abstract     Background. Several epidemiologic studies have shown that fibrinogen is involved in atherogenesis and that its predictive value is comparable to that of conventional risk factors. Fibrinogen level is a major determinant of cardiovascular risk in persons with hypercholesterolemia, hypertension and diabetes. Clinical cohort studies have also demonstrated that fibrinogen might be an indicator for the sequels of cardiovascular disease. At present, knowledge of the mechanisms through which fibrinogen is involved in the atherogenesis is fragmentary. Fibrinogen affects blood coagulation, blood rheology, platelets and monocytes-macrophages aggregation, and in addition it has direct effect on the vascular wall. All of these interactive pathophysiological mechanisms are involved in the atherogenesis. The role of fibrinogen in atherogenesis is supported by close association between fibrinogen concentration and cardiovascular events. Conclusions. Even though many crucial question remain unanswered, on results of several studies it is concluded that fibrinogen is related to cardiovascular diseases and to the prevalence of coronary, cerebral and peripheral arterial occlusive disease.
Summary     Izhodišča. Številne novejše epidemiološke študije ugotavljajo, da je raven fibrinogena pomemben dejavnik tveganja za razvoj ateroskleroze in da ima primerljivo napovedno vrednost kot običajni dejavnik tveganja. Koncentracija fibrinogena je eden od kazalnikov kardiovaskularne ogroženosti pri bolnikih s hiperholesterolemijo, hipertenzijo in sladkorno boleznijo. Klinične študije so tudi pokazale, da je fibrinogen lahko kazalnik kardiovaskularnih zapletov pri že napredovali žilni bolezni. Današnje razumevanje vpletenosti fibrinogena v aterogenezo je nepopolno. Fibrinogen vpliva na koagulacijo krvi, na reološke lastnosti krvi, agregacijo trombocitov in monocitov/makrofagov. Poleg tega ima neposredni vpliv na žilno steno. Vsa ta medsebojno se prepletajoča učinkovanja so sestavni del patofizioloških dogodkov v aterogenezi. Vlogo fibrinogena v aterogenezi podpira tudi tesna povezanost med koncentracijo fibrinogena in kardiovaskularnimi dogodki. Zaključki. Čeprav ostajajo nekatera ključna vprašanja nerazjasnjena, lahko zaključimo, da so izsledki dosedanjih študij pokazali povezanost med koncentracijo fibrinogena in kardiovaskularnimi boleznimi in da je le-ta povezana s prevalenco koronarne, cerebralne in periferne arterijske obliterativne bolezni.
