Abstract     Patient with critical chronical ischaemia (CCI) could be treated either surgicaly or with techniques of the invasive radiology. High risk patients could be treated also by different conservative or semiconservative means, but results are doubtfull. A high risk patient with CCI and three weeks old myocardial infarction is presented, where epidural analgesia and simpaticolysis were used to overcome eight months time interval, after which patient underwent successfull surgical reconsructive procedure. Two times daily 10 ml 0.16% bupivacain with 3 mg of methadone were administred. There were no complications during the months interval, except the unintentional (three times) removal of the cathether. It is concluded that epidural simpaticolysis and analgesia are the treatment of choice in high risk patients with CCI but without the wet gangrene.
